Holly, LPN, is an exceptional visit facilitator whose professionalism and dedication consistently enhance the quality of care we provide. During a recent home visit to complete HEDIS measure gap closures, Holly recognized that the patient was not doing well. Although the visit’s primary purpose was routine, Holly’s clinical judgment prompted her to look beyond the checklist. She immediately contacted the primary APP, clearly communicating her assessment and concerns. The APP agreed with Holly’s evaluation that the patient required prompt medical attention.
Holly’s ability to identify a concerning change in condition and act without delay demonstrates outstanding clinical awareness and patient advocacy. Holly is a go-getter and regularly goes out of her way to make her colleagues and our patients feel loved. She never complains about the distance or the miles she travels to see someone in need. She loves to bake and always brings homemade pepperoni rolls (a WV food staple), muffins, or cookies to staff meetings and one of my widowed patients.
Holly and I have been working together to care for a gentleman with advanced kidney cancer who is gravely ill and lives alone. When she arrived for a scheduled visit, and he didn't answer the door or reply to her texts, she stayed put. Holly was prepared to call law enforcement for a welfare check and refused to leave until we were able to make contact with a family member to ensure he was safe. He was touched that someone from WellBe who had only met him once before would care about his well-being so deeply.
