Had Kelly waited and not had the patient's best interest at heart, there may have been an even worse outcome.
Kelly White always goes above and beyond for her patients and her peers. As Kelly’s manager over the past few years, I have watched her grow into a confident, skilled, compassionate caregiver. Kelly truly leads by the love she has for what she does and for those she serves. The care that she provides is excellent.
Home health can be particularly challenging. Clinicians often feel alone and isolated. Kelly uses all of her resources wisely and effectively to make the connections she needs to support herself in providing the best care. Kelly’s peers often give her shout-outs for her excellent communication with her patients, the care she provides, and their progress.
Most recently, Kelly was assigned a patient who had a challenging hospital discharge. The patient had a respiratory diagnosis and was sent home with an RX for a nebulizer, and was told it was ordered. The patient initially did not have a home health referral and did not get a follow-up call from the hospital to check in regarding their discharge and if help was needed. The patient was seen on a Sunday by a home health nurse, and the nebulizer had not been ordered. The admission nurse followed up but did not get a resolution that it had been ordered until Monday.
The patient had now been home about 5 days without this medication. Kelly was assigned the next day to follow up on the nebulizer and instruct the patient on how to use it. Initially, Kelly was going to reschedule the visit as the nebulizer had not yet arrived, but her instinct told her to make the visit anyway. When Kelly arrived at the patient’s home, the patient was in significant respiratory distress, which required Kelly to send the patient to the Emergency Department. The patient was then admitted to a step-down unit. Had Kelly waited and not had the patient's best interest at heart, there may have been an even worse outcome.
