The Kent & Sussex RN Care Managers exemplify all of the 5 core values of Bayhealth. It is with immense pride and admiration that I nominate the RN Care Managers Team for their unparalleled commitment to excellence in discharge planning, quality initiatives, and utilization management. This team exemplifies the highest standards of nursing practice, ensuring that patient care extends beyond hospital walls to support seamless transitions, superior outcomes, and sustainable resource management.

In discharge planning, the RN Care Managers have consistently demonstrated exceptional dedication to preparing patients for successful recoveries. Through personalized education, collaboration with interdisciplinary teams, and timely coordination, they address each patient's unique needs, fostering continuity of care. Their proactive approach reduces readmissions, enhances patient satisfaction, and supports holistic healing.

Equally remarkable are the quality initiatives spearheaded by this team. They have played a pivotal role in driving innovations that elevate healthcare standards, including implementing evidence-based practices, streamlining workflow processes, and championing patient safety measures. Their unwavering focus on improvement has not only benefited patients but also set a benchmark for excellence in the healthcare community.

Last year, the team had 2 RN Care Managers honored as Delaware Top Nurses, with one RN Care Manager actually winning the category. This year the team was proud to recognize that their Director and Clinical Manager were both honored to receive nominations in the category of Leading & Advancing the field; one RN Care Manager received dual honors with nominations in Nursing Professional Development & Case Management; one RN Care Manger received a nomination in consecutive years (2024 & 2025); one RN Care Manager was recognized in her Ambulatory work as a Post Acute Administrator; one RN Care Manager received a nomination for Volunteerism, and two additional RN Care Managers received their nominations in Case/Utilization Management. Also, there are currently 12 RN Care Managers holding certifications: an achievement of 38% of the team. Study sessions are underway to grow that number even more.

Furthermore, in utilization management, the RN Care Managers have showcased an acute awareness of resource optimization while prioritizing patient welfare. By meticulously assessing care plans, navigating complex healthcare systems, and advocating for appropriate interventions, they ensure that both clinical and financial resources are employed judiciously.

The achievements of the RN Care Managers are a testament to their extraordinary skills, compassion, and teamwork. Their unwavering dedication inspires peers and elevates the entire institution's standard of care.
