Christina expressed gratitude and understanding when he told her that he had returned to use, ensuring to focus on harm reduction and compassionate recovery communication to decrease stigmatization.
A veteran on the SUD-RRTP did not return from off-campus privilege and remained on unauthorized absences, not returning for two days. Christina worked the day shift on the MHRRTP and reached out to the Veteran via phone. She was able to assess the veteran's status of safety, mood, and possible return to use. Due to Christina's exceptional therapeutic communication and established level of trust, the Veteran felt comfortable informing Christina that he had returned to use ( Fentanyl, Cocaine, and Alcohol). Christina expressed gratitude and understanding when he told her that he had returned to use, ensuring to focus on harm reduction and compassionate recovery communication to decrease stigmatization.
The veteran did state he would be on his way back. The veteran did not return to the unit, and when Christina returned the next day, she again reached out to the veteran. The Veteran did call the unit back and stated he was on his way back. Christina then notified the Clinical Coordinator and MOD to prepare for the Veteran's return and work on a care plan. The Veteran was requesting to go to 4 Lower for a day or two detox. When the Veteran returned and was informed that he would need to go to CDH for medical clearance, he refused. Christina and the MOD created a plan of care and allowed the Veteran to remain on 4 Upper with CIWA and COWS assessments, increased safety checks, and calls to the MOD if the Veteran's symptoms worsened. Christina educated all RN's on the unit through emails, teams, and shift change reports to ensure Veterans' safe monitoring continued. The veteran remains in treatment and will complete the program and the planned aftercare.
