Patient Advocacy at its best!!!A Patient was hospitalized in February for acute respiratory failure and was prescribed Pulmozyme as part of the treatment regimen. The patient was seen in the office subsequently in March and August, where Pulmozyme was continued to be prescribed. Prior authorization for Pulmozyme was initially filed with BCBS, subsequently denied due to the FDA indication being cystic fibrosis only; on the contrary, the patient was prescribed Pulmozyme for chronic respiratory failure and chronic cough. Bethany with her tenacious attitude continued to follow-through and was able to confirm with Genentech Foundation that they will supply patient with medication for off-label use only if the patient meets income requirements. When speaking with the patient's mother, she said that UHC had been covering the medication in the past. She subsequently called CVS Specialty Pharmacy, Monroeville, to confirm. After speaking with the pharmacist, who stated that the patient's account was marked for self-pay and had a balance of $8,187.34. The pharmacist stated that they usually let patients know about costs when they schedule shipments, but could not provide documentation where this information was given to the patient and or/patient's mother. She was informed that the patient's Pulmozyme was being "billed on the back end through paper billing". Bethany immediately asked for escalation on this case due to the oversight by CVS to inform the patient of the high costs and shipping medication regardless. The pharmacist was able to contact other CVS employees and eventually came to the resolution that the patient would be billed through UHC from now on, and their copay would be $0. The patient still had an $8,187.34 balance for the previously received medication. She stated that the patient would need to consult with insurance to see if they will back pay. Bethany asked for a call reference number, but CVS was not able to provide one. Bethany was able to confirm that the call was recorded and would be able to be referenced if there were further billing issues. The next order was delivered to the patient the following week without any incident. The patient's mother was informed of the situation and verbalized understanding. Bethany stated that the Patient's mother would continue to follow up with insurance and pharmacy. Bethany encouraged her to reach out to her if she had any further issues or concerns.
