We want to nominate the NSG 6 South Team for the DAISY Team Award. There are many exemplars specific to the commitment they prioritize regarding patient experience and safety while embodying what it means to be an extraordinary team of nurses (and essential healthcare workers). A patient was admitted to our unit over 300 days ago. The patient is not alert and oriented, is moderately immobile, and has significant comorbidities, making their care plan quite complex. With a general demeanor of impulsivity, this patient requires frequent reorientation. Throughout the days and months this patient was admitted, the staff built on the foundations of caring and compassionate care and frequently visited their room, even if not on their direct assignment for the shift. The family lives out of state, so this particular patient started displaying signs of apathy and depression a few months into their hospital stay.
The staff would inquire about fond memories and significant milestones in the patient's life to cheer the patient up. Upon doing so, staff recognized the importance this patient had on physical appearance and recognized that due to the patient's transgender status, this was of concern to the patient as days/months passed. The patient arrived at the unit with acrylic nails, which they loved. They were bright in color and were something the patient could look at to feel good. As time passed, the acrylic and natural nails underneath grew in length, and some began to fall off. This upset the patient as they were something they were proud of.
Day shift supervisor Tenaya Swiden arranged for the patient to receive a manicure while in the hospital after approval from the providers. The remaining acrylic nails were removed, and the natural nails were treated and polished for a fresh and symmetric appearance. This touched the patient greatly, and the staff continued to visit and compliment the appearance of the nails to boost the patient's confidence. Though the hand nails were treated, the toenails actually began to grow in length, almost curling around the toes and as this was a potential safety event; nurse clinician - Caitlin Shoemaker admirably and daily requested an updated plan of care to treat the patient's growing nails. This meant escalating through the chain of command during her shifts by requesting a podiatrist consult with any provider on the case that would consider the approval of placing the order.
Ultimately, the patient received the Podiatry consult, trimmed their nails, and a potential safety event was avoided. With a determined effort focused on safety, comfort, and patient experience, the 6 South Team worked together seamlessly and wanted the patient to get the best standard of care we could offer. In the upcoming week, the 6 South Team is excited and honored to be celebrating this patient's birthday with them. They look forward to making the day special with balloons, cake, and laughter. Northside's Mission and Values are consistently prioritized within our care and expressed through this story. Through compassion, teamwork, and service, we made a difference in our patients' lives and fulfilled our duties to serve and be present for patients in challenging times and support those patients who may not have a support system of their own.
