The DAISY Team Award
Extraordinary compassionate care is often the result of people coming together for a patient or family when they need it most. The DAISY Team Award recognizes nurse-led teams of 2 or more whose collaboration, dedication, and compassion create a truly meaningful impact on the care experience.
This Award celebrates a specific group of individuals who worked together to make a difference for a patient, family, or patient population. Whether navigating a complex clinical situation, supporting a family through a difficult time, or going above and beyond to meet a unique need, these teams demonstrate the power of compassionate teamwork in action.
The DAISY Team Award will complement your on-going DAISY Award program that honors individual direct care nurses for their extraordinary compassion and skill. If you choose to celebrate nurse-led teams, we ask you to present only one DAISY Team Award per year, in addition to the DAISY Awards you regularly present to individual nurses. The team must be led by a direct-care nurse.
Eligibility and Criteria
The DAISY Team Award is intended for a defined, nurse-led team that:
- Directly impacted the care of a patient, family, or patient population
- Includes nurses in a leading or central role
- Worked together to provide extraordinary, compassionate care
- Demonstrated collaboration, communication, and a commitment to putting patients and families first
Examples of Team Award nominations
- Creating special moments and compassionate environments for patients. Example.
- Going above and beyond to connect a patient with services, care, or additional needs. Example
- Putting together a wedding, anniversary celebration, birthday party, prom, or other event for a patient or family member. Example.
- A patient of family member nominating several nurses who made an impact. Example.
A note about team size
The DAISY Team Award is designed to recognize a specific team whose compassionate actions can be connected to a patient or family care experience. Because of this, the Award is not intended to honor an entire unit, department, or organization-wide group unless they directly impacted the care being described in the nomination. For example, an entire organization that came together during a natural disaster to evacuate patients certainly qualifies for The DAISY Team Award, but raising funds for food for a local event does not. Every person who receives the recognition should know why they were nominated and how they made an impact on a patient or family member.
We know that exceptional care happens every day across entire units, and we deeply value the contributions of all those who make that care possible. The purpose of the Team Award, however, is to spotlight a distinct nurse-led team whose collaborative efforts made a remarkable difference in a particular patient or family story.

Collecting Team Nominations
We recommend two ways of generating nominations:
- You probably already get nominations for The DAISY Award that include two or more nurses or nurses plus other members of the care team who have done something truly special for a patient or family. Separate these from your nomination "pool," and hold them for your annual Team Award.
- Once a year, issue a call for nominations to your staff.
DAISY Team Award Recognition
Once you have identified the team you want to honor, complete the order form in our online Resource Center so we can create a special DAISY Team Award plaque, customized with the Team's Name (i.e., Julia's Angels, Team NICU Night Nurses, etc.). Please plan ahead for this celebration. Plaques can take up to 4 weeks to create, and orders are shipped once payment is received.
In addition to the plaque, each member of the team will be recognized with a personalized DAISY Team Award certificate. (We will email you the Team Award certificate template for your use when you order your Team Award package.) The Team Award package also contains up to 10 DAISY Team Award pins and one certificate portfolio. Of course, you may purchase more if the team is larger than 10 members.
We hope you will celebrate the Team you select to be honored with The DAISY Team Award with a ceremony similar to those you conduct to celebrate your individual direct-care DAISY Nurse Honorees.
Ordering Your DAISY Team Award Package
If you have a team you want to recognize with The DAISY Team Award, you may order a DAISY Team Award Package by completing our Team Award Order Form in the Resource Center. If you need help logging into the Resource Center or placing your order, please contact your Regional Program Director.
