The Inpatient Rehabilitation Unit at Farmington Hills Hospital is a true example of interdisciplinary excellence. This team—nurses, nursing assistants, PPS coordinators, a social worker, therapists, and two physician groups—works seamlessly to restore independence and improve lives.

Patients are evaluated daily by therapy and reviewed twice during their stay in interdisciplinary rounds, ensuring timely updates, family involvement, and safe discharge planning. These efforts have reduced the unit’s average length of stay to 11.9 days, outperforming the national average of 12.6 days. Discharges home have risen from 76.8% last year to 80.3% this year. Key contributors drive these results.

Social Worker Karen Currie strengthens discharge planning, while PPS Coordinator Leanne Schoenbeck ensures smooth admissions and insurance approvals. PT/OT Leadership and PPS Coordinator Carla Bono improved GG score compliance, enhancing outcomes in mobility and independence. Therapy now completes same-day evaluations, supported by nursing for timely transport. Carla’s precision in IRF-PAI completion achieved 99.2% data accuracy, surpassing the national average.

Leadership has been transformative. Since February 2025, Nurse Manager Danielle Donner, BSN, RN, has fostered trust, collaboration, and innovation. Under her guidance, patient experience scores rose 12%, monthly discharges grew 22%, length of stay decreased by 1.2 days, and census increased 18%.
