It is with great admiration and gratitude that I nominate Nurse Kemesha for the DAISY Award. As a dedicated member of the Referral Coordination Team, Kemesha consistently exemplifies the highest standards of nursing excellence, compassion, and patient advocacy. In a role that often operates behind the scenes, she has become a cornerstone of our care coordination efforts. She ensures that Veterans receive timely, appropriate, and seamless access to specialty care, whether within our facility or through community partnerships. Her attention to detail, clinical judgment, and unwavering commitment to patient-centered care have made a profound impact on both patients and colleagues.

One particular instance stands out that truly captures Kemesha's dedication. A Veteran with multiple chronic conditions had been referred for a complex specialty consult that required coordination across several departments and community providers. The Veteran, who was also experiencing housing instability and transportation challenges, was at risk of falling through the cracks. She not only recognized the urgency of the situation but also personally followed up with each provider to ensure the consults were scheduled appropriately and even collaborated with social work to arrange transportation. The Veteran later shared that it was the first time in years they felt truly seen and cared for, not just as a patient, but as a person. This is just one example of the many ways that Kemesha goes above and beyond. She takes the time to listen to Veterans' concerns, explain referral processes clearly and reassuringly, and advocate for services that align with each Veteran's unique needs and preferences. This level of personalized care not only improves outcomes but also builds trust and confidence in the healthcare system.

Beyond clinical excellence, Kemesha is a collaborative team player who fosters strong relationships with providers, schedulers, and administrative staff. She is always willing to go the extra mile, whether it's troubleshooting a delayed consult, coordinating urgent care for a high-risk patient, or mentoring new team members with patience and grace.
