Through consistent teamwork, proactive rounding, and shared accountability, the Memorial 3 team most recently achieved 95 consecutive days fall-free, demonstrating their unwavering commitment to patient safety and individualized care.
I am honored to nominate the Memorial 3 Team for the DAISY Team Award in recognition of their exceptional commitment to excellence, compassion, and innovation in patient care. Over the past year, the team has demonstrated remarkable resilience and unity as they transformed their nursing practice to better meet the individualized needs of their patients. Grounded in the UHS values of Compassion, Teamwork, Trust, Respect, and Innovation, the Memorial 3 team successfully developed and implemented a team-based nursing care delivery model. Their goal was clear: ensure that nursing support and patient care consistently lead to the highest-quality outcomes.

Despite entering this journey during a period of great uncertainty, the team not only adapted—they prospered, strengthened their professional bonds, and fully embraced a collaborative approach that elevated the care environment for both patients and staff. A key component of this transformation was the team’s dedication to learning and integrating the LPN scope of practice within their daily workflow. RNs and NAs engaged wholeheartedly in education, skill-building, and shared learning. They participated in the hiring process, precepted newly hired LPNs, and intentionally shaped a team-centered routine that fostered consistency, communication, and mutual support. The transition was not without challenges. The team navigated changes in task sharing, assignments, and documentation processes—areas known to test even the strongest teams. Yet, through open communication, respect for each other’s roles, and a collective commitment to excellence, these obstacles ultimately strengthened the team’s morale and solidified their trust in one another.

Most importantly, the impact of this work is clearly reflected in improving quality indicators and patient outcomes. Through consistent teamwork, proactive rounding, and shared accountability, the Memorial 3 team most recently achieved 95 consecutive days fall-free, demonstrating their unwavering commitment to patient safety and individualized care. Patients benefited from more coordinated care, enhanced communication among caregivers, and a more supportive and engaged nursing team at their bedside. The Memorial 3 team exemplifies what it means to live the UHS values each day. Their compassion for patients, commitment to each other, and willingness to innovate make them incredibly deserving of the DAISY Team Award. Their journey is a powerful example of what can be achieved when a team comes together with purpose, resilience, and heart. For these reasons, I am proud to nominate the Memorial 3 team for this well-deserved recognition.
