Rebecca did the right thing and advocated with clear intentions to help this patient live the healthiest life possible.
There was a "younger" patient who had debilitating neurological symptoms. These changes came on quickly, leaving the patient and their significant other feeling unprepared and scared, especially regarding discharge.
The discharge plan was set for the patient to go home with possible home health. When Rebecca entered the patient's care team, she immediately connected with the patient and spouse. Rebecca identified that going home wasn't a safe discharge plan. She advocated for the patient and spouse vigorously to find a safer alternative.
After communicating with the nurse manager, patient nurse, and provider, Jessica reached out through an unexplored path to one of our community facilities. Rebecca's desire to do what's best for people helped navigate this alternate discharge plan, and the patient was discharged to a facility where recovery and safe care are more likely.
