We had a patient who was homeless and had frequent visits to the hospital due to a deteriorating medical condition. The patient was unsafe to return to the previous homeless environment, so the team brainstormed to develop a safe discharge plan. The team located the patient's family, and, in collaboration with our Care Management Team, resources were provided to help transport the patient to a safe place after discharge.

It was uncovered through communication with the family that there was a family history of Huntington’s Disease, which was characteristic of the patient’s symptoms and groundbreaking in the patient’s medical treatment.

The team advocated for the patient and diligently worked to find family to support him, ultimately resulting in his reunification with his family in Mexico.

I am so proud of this team, as well as the outlying vital team members who assisted with this patient.
