By the end of the patient’s nurse visit with Elisha, the patient had received a new blood pressure cuff and log for home monitoring, a review and adjustment of his blood pressure medications, lab orders for further assessment, and follow-up appointments with both his PACT provider and his PACT PharmD.
Elisha goes above and beyond in the daily care she provides to Veterans. Not only is she compassionate, but she also displays excellent, clinically competent nursing care with the patients and families she serves as a Registered Nurse at Charlie Norwood VAMC. When a veteran presented to the Primary Care Clinic as a walk-in requesting a refill of his blood pressure medication, Elisha quickly reviewed the patient’s record and assessed his needs. During her assessment, she realized the veteran had been taking a total of 6 blood pressure medications, prescribed by different providers. Elisha educated the patient on the importance of reconciling his medications. Elisha initiated a nurse visit and performed a blood pressure check on the patient. Despite taking all 6 blood pressure medications, his blood pressure remained elevated. Elisha provided further education on proper blood pressure monitoring, hypertension medication management, and the importance of regular follow-up with his primary care provider. Elisha also alerted the patient’s Primary Care Provider and pulled in the PACT PharmD. By the end of the patient’s nurse visit with Elisha, the patient had received a new blood pressure cuff and log for home monitoring, a review and adjustment of his blood pressure medications, lab orders for further assessment, and follow-up appointments with both his PACT provider and his PACT PharmD. Not only was the patient extremely grateful, but the PACT PharmD summed up her gratitude with the following message: “There’s no other way to say it: We’d be lost without you." Thanks for always being a great team player! Having Elisha see the walk-in nurse visits on Team E has been incredible for streamlining. She is fantastic when I have a patient with symptoms that may need to be screened by the PCP. She is helpful even on her busy days. Team E and especially our veterans are so lucky to have her on our team!
