She is loyal, dedicated, smart, kind, hard-working, and totally amazing.
Sophea is the epitome of an employee living the values of Intermountain Health. Two weeks ago, and again yesterday, Sophea had the entire floor, 32 patients, on her own. She works tirelessly to make sure all of the patients' discharge needs are met. She never complains and always makes sure things are done.
Our new social worker thought coming to T10 would be a break from the floor she was on. After coming to T10 and seeing how busy it was, she realized that our floor was not easier but very hard. She soon learned that it was Sophea, never complaining and always going the extra mile, who made others think our floor was easy.
Yesterday, it was five minutes until the time for her to get out of work. We had a new patient who had stated she was feeling suicidal. Even though it was quitting time and Sophea is a case manager and not a social worker, she made sure she got the nurse in contact with the on-call social worker, and made sure all the documentation was done and the patient was safe with a sitter before she left for the day.
Sophea makes a weekend case management report for T10 every weekend. She puts all the patients who are expecting to go over the weekend and what has been set up for them. She also puts any discharge needs, and if we still need to contact the weekend case manager. She makes a transplant patient report every weekend, stating who has been seen and who still needs to be seen for their transplant case management discharge information.

Sophea works every day to make sure the patients' discharge needs are met. The patients are able to go home knowing that all of their home health needs have been taken care of.
She works with facilities to make sure patients' insurance is accepted and that they have a bed for the patient. She communicates with family members to make sure everyone is in the loop with the next steps for the patient. She consistently helps the doctors as they come to her with last-minute requests for the patients. Whenever there is a patient crisis/issue/event, she makes sure the family is okay without being asked.
