Mary has been the go-to for professional practice for many years. During her tenure, she has assisted in expanding her space by 400%, during which she has ensured high quality and professionalism. She has this same drive for high quality in her personal work. We see this in all her projects, but especially in the Heart Failure Clinic.
The Heart Failure Clinic was officially created in 2021 with the intent to be an integrated treatment area to improve heart function in patients. The KPCO members learn what CHF is and the important self-care skills that will improve quality of life. The team consists of APPs, RNs, and an LPN who work together seamlessly to improve heart functioning and decrease heart failure-related hospitalization.
The members progressed bi-weekly by phone with frequent interventions based on those check-ins. Mary's partnership on the heart failure protocols allows autonomy for the RN to titrate the medication with appropriate delegation and follow-up, directly supporting the aim of getting these patients to the maximum tolerated dose faster than if otherwise managed.
This has directly impacted the quality of care for these members. Mary has not only assisted with titration protocols but also with procedures that the entire team utilizes to ensure efficient and high-quality care for the member at every interaction. She also ensures the quality continues with frequent check-ins with the team and updates to the documents as they are needed.
This nurse leader guides not only her peers but also leads by example in her medical specialties. We are so fortunate to have such an incredible nurse leader here at KPCO.
