Subsequent acute care took place, and Adriane proceeded to engage in intensive care coordination with the Office of Community Care in the VA, community care providers such as Oncologists, Surgeons, and various other professionals, including ostomy care. Adriane provided daily ostomy instruction for the Veteran, given her strong relationship and rapport with the Veteran, who has a severe mental illness and challenges with engagement with people he does not know. Her unwavering dedication to her patients and the VA mission is clear in her tireless efforts to improve the lives of those she serves.
