Francisco had a very complex case involving a patient who had several failed discharges and readmissions. Patient required extensive care both during his hospitalization and at home upon discharge. Patient was requiring DME and Home Health Trach, Vent, Suction, Wound Vacs, Ostomy and additional Wound Care needs to safely and successfully transfer the patient to home under the care of the family.

Coordination of Care, DME, and teachings for the family was no small undertaking; this discharge plan took a village to complete, and Francisco was a key member of the team. He took it upon himself to make sure that all parties knew what was needed and kept track of items completed to ensure a successful discharge. Additionally, this RN provided a listening ear for family and his team members, whether to vent or to just have a safe place to process the events taking place around the patient's care.

He continued to show compassion and empathy that went above and beyond what most people would have been capable of during this time. He continued to show up for the patient, family, and his team every shift without hesitation, and took charge of the next steps that made all the difference in making this a truly successful discharge. He even made himself available after the patient was discharged to the home team, providing information that avoided what would have been another readmission.

His patience and kindness are something we all should strive for and be envious of.
