Laura McNamara, Nurse Practitioner of the Palliative Care Team, was an incredible guide to our critical care team in providing culturally and spiritually sensitive care to a Native American veteran and his family during his transition to end-of-life care. The patient's son had been released from active duty to be with the veteran at the end of his life. The veteran's son had not seen his father in a long time and needed time to adjust to his father's condition and prognosis. Laura was sensitive to the children's need to be present and together at the bedside with their father. I observed how Laura demonstrated the gift of presence on several occasions, being present with the veteran's children as they sat in silence with him and wrestled with the very difficult decision to transition him off ventilator support. Laura was able to skillfully read when the family was not ready and then when they were ready to discuss this very difficult decision. Laura demonstrated patience in not pressuring the family with a timeline in which they needed to make a decision. The family requested that the patient be transferred to a hospital room where the window could be opened prior to his passing so that his soul would be able to depart. Laura arranged the coordination of a room on the Progressive Care Unit where the window could be opened by Clinical Engineering and helped to facilitate the smooth transfer of the patient to this room so that the window was already open when healthcare staff transferred the patient out of the Surgical Intensive Care Unit while the family waited in the waiting room. Laura was an amazing guide to nursing staff with her recommendations on the best medications and timing to use to keep the veteran free of anxiety and comfortable, which was his family's main concern as he was gradually titrated off ventilatory support. I am so grateful that Laura McNamara, NP is a member of the Palliative Care Team and that she was present at the bedside the entire time during the patient's passing to support his family and nursing and respiratory therapy staff. She has a calm, kind, genuine, soft-spoken demeanor and should be honored for the amazing and compassionate work she does every day.
