Danielle is known for going above and beyond in advocating for her patients. She recently had a patient who had been estranged from all of his family members. Because he was fairly confused, he was unable to make his own arrangements for the care he would need following discharge. Danielle collaborated with the social work team to find the patient's next of kin.

After an extensive search involving several community partners, they obtained contact information for this gentleman's adult son. His son lives with a developmental delay and requires a caregiver. This caregiver shared that the patient's son has been asking about his dad for years, but had no idea where to find him.

The patient was nearing the end of his life, and as Danielle was helping arrange long-term care and hospice, her patient shared with her that he had no family or friends to notify. Danielle was able to tell her patient she had located his son. The patient said he would love to have the opportunity to talk to his son, whom he hadn't seen in at least 20 (maybe 30) years.

Danielle worked with the son's caregiver to arrange a video call for both of them before discharge. They had a pleasant conversation, and both were happy to reconnect. Danielle genuinely cares for the person, not just the patient, in front of her. This clearly comes naturally, but she truly goes extra mile.
