Even when we encountered significant roadblocks that could have halted the transfer, Aaliyah and I worked closely to develop alternative solutions and compile supporting evidence, which she promptly brought back to the team.
I wanted to take a moment to express my sincere appreciation for the outstanding support I received from Aaliyah Love, Care Coordinator, during a very complex patient care situation.
Aaliyah was a vital component in helping manage the case. This patient needed both a liver and lung transplant, which could not be performed at VCU Health; however, a hospital in Florida had the capability to save his life.
As his primary care nurse, I discussed the plan of care and my concerns about his rapidly deteriorating condition with Aaliyah. At the time, the outlook for transferring him was not promising due to the complexity and cost. Aaliyah listened intently, never once dismissing my concerns, and took the initiative to escalate this to her leadership team. She advocated tirelessly on behalf of the patient and kept me informed throughout every step of the process.
One standout moment was when Aaliyah took it upon herself to orchestrate a family meeting. She brought together the patient’s mother and brother, the attending physician, her director, the team lead, and the unit social worker to collaboratively address the situation. Her ability to coordinate this meeting so swiftly and effectively not only provided much-needed clarity and support to the family but also played a pivotal role in moving the transfer process forward.
