Jac worked closely with case management over several days to close communication loops, understand the barriers, and help effectively plan a safe discharge avenue for this patient.
I am nominating Jac on behalf of some of our ICU staff members for the DAISY Award due to her extreme diligence in arranging care for a patient in our ICU. We recently had a patient admitted to our critical care unit with significant psychiatric needs that required around-the-clock monitoring and care. Jac was noted by her team to be extremely engaged in the care planning of this patient. Jac worked closely with case management over several days to close communication loops, understand the barriers, and help effectively plan a safe discharge avenue for this patient.
Jac, during that time, also worked very closely with our risk management team to ensure the safety of the staff and the patient was accounted for each shift. This required out-of-the-box thinking and creative solutions to ensure the unit and the patient would be set up for success. When the patient was able to be safely discharged, multiple members of our ICU team and risk partners stated, “Jac worked really hard to make it happen.” Through this situation, Jac also supported her team and the unit so that no staff member or other patient’s needs were left behind. Jac, thank you for your hard work in navigating a challenging situation that doesn’t often happen in our unit. Your attention to the safety of the patient and the team did not go unnoticed, and I hope you know how much all of us appreciate the work you do every day to ensure we provide the safest care possible. You are an amazing charge nurse!
