Recently, this nurse identified and addressed significant ethical risks in the patient referral process to primary care providers in the clinic. Many of these patients are frequent ED users, often readmitted due to a lack of continuity of care by a primary care provider (PCP). Recognizing the complexity and inefficiency of the current referral process, she initiated crucial conversations with the medical director about the challenges faced by referring staff. Her relentless dedication and clear articulation of the issues secured collaboration and buy-in to recreate a new process. Her proactive approach led to a consensus among key stakeholders, resulting in a streamlined referral process that reduced the steps from seven to two. This transformation has greatly benefited the patient population, leading to improved access to care.
