Alicia was logged into the Neurology in-basket as scheduled. She received a My Chart message from a patient that concerned her. It described the patient not taking their prescribed medications and their behavior becoming irrational and suicidal, per the patient. Alicia reached out to the ANMs on site and at FHC, also adding the patient's MD to a secure chat for assistance. The Provider responded right away, giving Alicia instructions to contact the patient and check on her. Alicia called the patient, questioned her appropriately to assess suicidal or homicidal thoughts or ideations. While keeping the patient on the phone and communicating with the patient's Provider, the ANM called EMS/Police for a wellness check on the patient. Alicia kept her on the phone until the Police arrived at the home to assist the patient. Once the police arrived for a wellness check, they noticed the patient to be in physical distress with multiple medications on hand and in her presence. The patient was transported to the hospital for a mental health evaluation. The attention to detail and quick thinking Alicia showed resulted in a patient being kept free from harming herself and others. This showed great compassion, empathy, and teamwork on everyone involved to keep this patient safe.
