Rachiratou Tapsoba was caring for a patient when she noted upon assessment that the patient had abnormal bleeding that she was not notified about in report. The primary nurse was notified by Rachiratou, but the nurse was unavailable to attend to the patient at that time. She continued escalating her concerns and contacted the nephrologist and HSA. Rachiraotu’s persistence resulted in the charge nurse presenting to the suite to reassess the patient, where they found that the patient had profuse bleeding and a change in medical status that was not previously noted. A rapid response was called to ensure that the patient would get further evaluation, and the patient was transferred to the ICU. Rachiratou’s dedication and concern for this patient and all her patients ensure that they get the top-quality care that they deserve.
