Over the past couple of months, Elisha worked with a  DHS case worker, social work, community mental health, physical therapy, family, and care providers to provide effective case management to a patient with low vision. This patient’s goal was to remain independent and gain improved function while remaining in her residence. Elisha partnered with all above mentioned disciplines to provide the patient with a comprehensive educational experience during her time on home care, and to ensure that the patient’s needs were being met while also rising to meet each new challenge that the case brought.

In another recent case, our social work team was assisting a home care family in enrolling their loved one into hospice. It was a Friday evening nearing the end of the workday, and supports were in place, but the timeline was proving to be challenging. Elisha assisted social work via phone in assuring needs were met while awaiting approval for hospice admission. Elisha assists with the needs of patients with compassion and care, exemplifying the heart of nursing.
