She remained with the family to educate on the next steps with hospice and to provide comfort and advice on issues.
This nurse was recognized recently at Monmouth Medical Center for her critical thinking and perceptive home care skills, which aid in preventing rehospitalization. She is a compassionate caregiver to patients and family alike. This nurse has a wealth of knowledge that she shares freely with everyone. This nurse is often the first one that many patients meet, and her kindness, caring nature, knowledge, and effective communication set the tone for the patient’s service and outcome. She truly exemplifies all that the DAISY Award entails. VNA and our community are a better place because of this nurse.
Yesterday, this nurse arrived at SOC only to find that the patient was actively dying. She had an end-of-life conversation with the family (in English with the children and Sicilian with the wife!) She called VPS, and we had the hospice order and referral in process in a short order. She remained with the family to educate on the next steps with hospice and to provide comfort and advice on issues. The family asked if VNA would be sending a priest for last rites – she helped them to reach out to their priest. They asked if it was ok to call the patient’s grandchildren – she walked them through how it is ok to do whatever is best for the patient and the grandchildren. She isn’t only an amazing nurse, based on her knowledge and skill, but the care she gives to everyone involved goes above and beyond every day. It is my honor to present this DAISY Award to Paula Abel.
