Dan received a confused patient to the Dialysis unit. The patient was crying hysterically, talking to himself, mumbling incoherently, and talking to voices no one else could hear. Dan was told in report that the patient had been confused. After a thorough chart review, Dan found that the patient was on CIWA protocol and did a CIWA, getting a score of 17 on the patient!! He immediately followed the protocol and gave the allotted diazepam. The patient benefited from this almost immediately. After some more searching, it was found that no one had done a CIWA on this patient since admission (at least 7 days prior). He immediately told the Charge Nurse, and we were able to update the Team Lead on the floor that the patient came from, that this key piece of nursing care had been missed. Without Dan's astute dedication to patient care, this patient could have had detrimental side effects from his withdrawal symptoms. Dan provided exceptional patient care & was able to communicate with the proper team members to ensure this patient received the highest level of care.
