I would like to nominate Susan Abraham (and potentially, if you’re able to, the whole of 5 South) for their unwavering commitment to compassionate care in the face of danger while caring for a recently discharged complex patient. For over three months, Team Members on 5 South were faced with providing care to a complex patient. This patient's needs often placed Team Members in harm's way, resulting in physical and emotional injuries. With Susan's leadership, Team Members were instructed to place individual SafetyPause reports to help escalate the complications they faced. Additionally, Susan pulled together a multidisciplinary team to align the care plan as well as drive to multiple care conferences, which eventually led to a safe discharge plan. As part of this plan, Susan met with and spoke to the family on multiple occasions, spending time with the patient’s wife and mother, answering questions and calming anxieties as they moved toward a decision of care and discharge date. Lastly, and most importantly, Susan led by example, often providing care and stepping in to help during harmful situations to effectively keep other Team Members safe.
