Both Dad and I hugely appreciated your expertise, care, compassion, and dedication. A nomination for a DAISY Award is the least you deserve.
Nomination received from a relative who took the time to post in a nomination form after the passing of her father:
This nurse looked after my late father for many years – she not only looked after him but showed kindness and compassion to me, too. She always took the time to match up my Dad’s hospital appointments to limit the number of trips he had to make to the hospital – something that may seem insignificant to some, but this was a huge help to both my Dad and myself. She communicated regularly with me as his next of kin to ensure that both of us had a good understanding of his medication –especially if there had been any changes made at the clinic.
The nomination also included a personalised message, which reads:
“Dear Nurse,
My lovely Dad enjoyed going to his appointments because you made them so easy for him. You coordinated his bloods and scans and always offered to see him to suit his other hospital appointments. I only met you on one occasion but I feel like I’ve known you all my life as we spoke on the phone so often. I had no qualms about him attending his appointments without me because I felt reassured that you would phone me if you had any concerns or of there was any changes made at the appointment.
Both Dad and I hugely appreciated your expertise, care, compassion, and dedication. A nomination for a DAISY Award is the least you deserve. Thank you from the bottom of our hearts for everything you did for us. We will be forever grateful. Our Lady of Lourdes Hospital is lucky to have you."
