Ms. Fronek triaged a call from a Veteran who requested an appointment in Primary Care to have his Foley urine bag replaced. When the Veteran arrived, she exchanged the foley bag but realized that he was incontinent of stool. The Veteran was wheelchair bound. Colleagues urged her to take him to the ED to be cleaned up. She knew this was not the most sensible action. With a peer’s assistance, she used a ceiling lift to help get him cleaned up and completed a skin assessment.

Ms. Fronek contacted a Veteran who was a no show for his planned admit for alcohol abuse. When she reached him, they connected over shared Air Force experiences. She learned he was fearful of being recognized. The Veteran presented to PC as a walk-in, with a plan to be admitted during an off-tour. She recognized he was demonstrating signs and symptoms of withdrawal and escorted him to the IMHAC clinic, and he was admitted. The Veteran later shared with her that he has been sober for 6 months, and he personally thanked her.

Ms. Fronek's dedication to excellence is evident in the disease management she provides. She initiated a team approach with Veterans with diabetes on her panel who did not achieve A1c goals. She partnered with a Whole Health Coach to provide information on daily diets, grocery shopping, glucose readings, exercise regimens, and weight changes. Four months later, these Veterans showed a full 1% decrease in A1c value. She also uses tele-health technology to promote tele-retinal screenings for Veterans with diabetes.
