Ryan’s sensitivity to these unspoken cues and his dedication to uncovering the whole picture very likely saved this patient’s life.
Ryan demonstrated exceptional compassion, insight, and clinical excellence while caring for a patient with Limited English Proficiency who was admitted to the EOU on the hypoglycemic protocol. The patient had initially presented to the ED via EMS after being found altered, with a blood glucose level of 49 mg/dL despite initial EMS intervention.

Upon transfer to the EOU, the patient declined the use of interpreter services. Through gentle persuasion and patient-centered education, Ryan was able to encourage the patient to accept the assistance of an interpreter. This critical step in improving communication allowed Ryan to uncover a significant issue in the patient’s insulin administration. By asking thoughtful, clarifying questions, and listening with both his ears and his heart—practicing what we call radical listening—Ryan discovered that the patient had been mistakenly administering his long-acting insulin at meals and bedtime, in addition to taking 30 units of Novolog right before bed. This regimen posed a serious and ongoing risk of life-threatening hypoglycemia.

Ryan also identified that both the patient and the patient's spouse may have been overestimating their understanding of clinical instructions or may have felt uncomfortable asking for clarification, highlighting a social or cultural barrier to care. Ryan’s sensitivity to these unspoken cues and his dedication to uncovering the whole picture very likely saved this patient’s life.
