I have had the privilege of working with Ms. Oberholtzer throughout the past year when arranging patient care and discharge planning. While I feel each patient received excellent care, there have been specific patient cases that I feel Ms. Oberholtzer has gone above and beyond to care for the patient and their family. One case in particular was a very difficult case to navigate as the patient had a highly complex medical history and some difficult ethical questions facing their continued care, ultimately leading to difficult family decisions. This case further proved to be particularly challenging from a case management standpoint when arranging outpatient care, follow up appointments, gathering supportive resources and assembling an overall comprehensive discharge plan. More so, this case was an emotional and stressful situation for both family and care providers. Ms. Oberholtzer continued to provide support and guidance for the patient and their family all while trying to coordinate discharge planning and assist with treatment/insurance coverage. She had numerous conversations with the patient's family to provide support and empathy, which strengthened the trust the family placed in the care team. Ms. Oberholtzer was also present to provide encouragement and support for members of the care team on particular difficult and emotional days, helping to maintain care team morale. I believe that Ms. Oberholtzer is a true DAISY Nurse.
