The patient was in the CEA and underwent a normal heart catheterization. They were requiring a medication through IV infusion that only the ICU can manage, and after the heart catheterization, the patient came to the ICU  in the afternoon and was managed by the ICU team, including night shift nurse Raceine Wilson. 

Two days later, on the night shift, Raceine went to say hello to the patient because they had created a rapport with each other, went to do her rapid response rounds, and came back to the ICU to chart and noticed the patient’s vital signs were showing a high heart rate, which Raceine said is not normal for this patient. The patient was an ICU patient, but had been downgraded to PCU level with a CRT PCU nurse taking care of the patient overnight. 

Raceine decided she was going to investigate further the high heart rate and took the ARNP Kelsie with her to see the patient at the bedside. Upon arrival at the bedside, the patient was in the recliner, noticeably diaphoretic, mumbling incoherently, and pale.  Both Raceine and Kelsie lifted the patient back into the bed and immediately moved the patient back to the ICU.

The patient was subsequently intubated, central line placed, started on 3 vasopressors and an EKG was done at the same time. The EKG showed an acute STEMI, where the doctor was called, and the STEMI team was activated. The patient was taken down to the cath lab, where the patient was shown to have a coronary artery perforation and had an Impella placed and a drain in the patient's chest. The patient also showed signs of a cardiac tamponade, and 300ml of blood was drained from around the heart. 

The patient was then transferred down to ORMC in the ambulance with the ICU team to assist in managing the patient. The patient arrived and was immediately taken to the OR for emergency surgery to fix the perforation in his heart. The ICU team also was unable to reach the patient’s wife by phone, so the team called 911 to make sure the wife was ok and to find out what was going on with her husband.
