These Nurses recognized a pattern of 9 patients within 4 months that had come in with a STEMI/PCI Intervention and were readmitted within 1 – 2 weeks with stent re-occlusion.

This project supports the MHTW Vizient Journey, specifically the readmission within 30 Days AMI-Focused. These patients were tracked real time, from our Chest Pain Coordinator’s watch list and repeat procedures within the 30 days.

This team recognized a serious issue within our STEMI population, and they felt they could make an impact and improve the outcomes of our patients. They did a deep dive into the identified patients to see if the issue was patient non-compliance, mechanical, or pharmacological.

The Nurses met with our Physician partners and came up with evidence-based recommendations. With the recommendations instituted, the outcomes were that it eliminated patients coming in with restenosis of a recent PCI.

Community: This project addressed a pattern of re-occlusion events that were occurring in STEMI patients receiving an intervention.

Compassion: This project prioritized the well-being of our patients, focusing on safety and support of those who have heart related issues. It actively seeks to address the needs of our heart population, fostering a sense of community and compassion.

Courage: The project encourages bold actions as our RNs came forward and embraced this challenge that required them to go to our Medical Director Dr. Avendano and CMO Dr. Vedala to research the common denominator causing the re-occlusion issue, all while empowering our team to get involved and make a difference.

Credibility: Safety is a cornerstone of this project, which is built on transparency, integrity, and accountability. The project was created to make improvements to our processes and to keep our patients safe.

Since August of 2024 when the identified recommendations were implemented, we have zero instant restenosis PCI cases related to mechanical or pharmacological issues.
