A couple of months ago, Ms. Hurth received a call from a Veteran’s wife, who had reached out to the provider but had not yet received a response. She was very distressed, expressing confusion about her husband's medications, unsure what to give, what to withhold, or how to administer them.
Ms. Hurth had previously met the Veteran and his wife while rounding on potential Home Telehealth Veterans. Due to the expressed confusion, Ms. Hurth decided to go to the Veteran’s home. She also messaged the provider and their team. When she arrived at the home, she found that the Veteran had multiple medications, and his wife did not know how or when to give them. He had 3 inhalers but did not know how to use them or why he had them. He has insulin per the sliding scale, but no sliding scale. He was missing two prescribed medications. A nebulizer was delivered; however, neither the Veteran nor his wife knew how to use the device.
Ms. Hurth was able to instruct the Veteran and his wife on the medications, the inhalers, and how to work the nebulizer. Ms. Hurth again followed up with the provider.
The Veteran and his wife were very grateful, and he is now on Ms. Hurth's panel for CCHT-RPM monitoring.
