Through persistence and empathy, Noemi transformed a potential care gap into a success story, ensuring a vulnerable veteran received the care and dignity she deserved.
Mrs. V, a 75-year-old Vietnam War veteran with a complex medical history, had recently endured a prolonged hospitalization for severe postoperative complications, including sepsis and respiratory failure. After more than a month in the hospital, she was discharged to subacute rehab, but with no follow-up appointments scheduled. Despite multiple outreach attempts by staff, Mrs. V remained unreachable, and her consults were discontinued. Many might have assumed she was non-responsive, but the nurse case manager refused to accept that.

Demonstrating unwavering commitment and compassion, Noemi Cosico persisted in outreach efforts, making repeated calls, leaving messages, and sending secure messages through My HealtheVet. Finally, Mrs. V answered and said, ˜Please text me. I am deaf." This simple statement revealed the root cause of her silence: profound hearing loss, worsened after hospitalization, had gone unaddressed. Rather than simplifying the issue, the nurse adapted immediately. She began communicating via text, completed case management intake, and coordinated care through secure messaging. She documented Mrs. V's communication needs in CPRS and informed the entire care team, ensuring continuity. 

Noemi reinitiated critical consults, scheduled follow-ups with PACT and Cardiology, and encouraged Mrs. V to engage with My HealtheVet. She even facilitated referrals to Mental Health, Audiology, and cochlear implant evaluation.

Impact on Patient and Care Team: Because of Noemi's persistence and sensitivity, Mrs. V's emergency department visits dropped from nine to three in six months. She regained trust in her care team, actively participated in her health, and expressed heartfelt gratitude for the team’s efforts to understand and accommodate her disability. Providers now use text messaging to adjust medications and coordinate care, dramatically improving her experience and outcomes.
