Michelle, we want to thank you for being such a wonderful, compassionate, and caring professional! On the reverse side of this form, you fit all of what it mentions. You cared for my physical, spiritual, and emotional needs with skill and confidence, taught me about my health and helped support my health decisions, truly listened to me and made sure my needs were met during my care, worked with me, my family, my support system, and my care team, to help me heal. You made me feel safe under your care, made me feel truly cared for and respected as a person.

You made a bad experience very good for us! You listened to everything we both had to say and were there for every need or question we both had!
