Mary Lacyk called a patient who triggered CHF and had been in the ED due to missed HD appointments three times. The patient was treated and released; when asked why he missed so many appointments, the patient stated that he missed his appointment due to transportation issues. The patient stated that he was disabled, had his identity stolen, had not had any money for 6 weeks, and was also facing possible eviction. He had little to no food in his home.

Mary provided advice for his rent issues and contacted a food bank and was able to obtain emergency funding to help the patient get back and forth to appointments and with food until his checks were reinstated. In the end, the patient was not evicted and was able to make it to appointments and had food until he got his checks reinstated. The patient has reached out multiple times to thank the Nurse Navigation Program. Mary saved his life!
