Her background as an oncology nurse, informatics, and education has been extremely valuable to our division and the organization overall.
At the beginning of the COVID pandemic, she quickly worked with various members of leadership, IS, and our division management team to create a robust tracking log for COVID patients that our hospitals were not able to discharge. This data was shared with the state which helped support the need and implementation of the state-supported "Super SNFs" to care for COVID patients in the community to free up our hospital's beds for patients who truly needed hospital level of care. She also advocated for our social service specialist team members to be eligible for the annual certification award. Her tenacity with this request was not only approved but also expanded for other ancillary disciplines as well. She is a creative and solution-focused leader. She developed, obtained approval, and implemented a medical respite program in partnership with our local Salvation Army shelters and BayCare HomeCare (Tampa and St. Petersburg). She recently expanded this program to the Winter Haven Salvation Army shelter. This partnership allows us to provide a safe discharge plan for our homeless patients who need continued care post-discharge. She is a team member advocate and is in the process of creating a pilot for members of our team to have the ability to partially work from home (a big WIN) for many of our team members. She was instrumental in hiring our division educator (a position that was vacant for over a year) and during that time, she organized and conducted our new hire orientation sessions. In recognizing a need, she sought approval (successfully) for our division to have a regional complex case social service specialist; this team member assists our core team/managers/supervisors with patients across the system who have complex discharge needs. She also manages our division preceptor program. In 2019, BayCare launched a large initiative with the Care Transition Rounds (CTR) implementation. Expansion of the CTRs into a weekend CTR is underway and I am proud to share that we are presenting the BayCare CTR process at a large National Case Management Conference in May 2022. She exemplifies the BayCare values in all of her interactions. She is always looking at ways to better improve our processes to support our BayCare Core Tenets.
