April 2026 Honoree
Kathryn Hoffman, DNP, RN, NE-BC
With more than 25 years of dedicated service to nursing, Katy Hoffman has devoted her life’s work to the compassionate, equitable, and evidence-based care of others. Throughout her distinguished career, she has exemplified excellence in nursing practice, leadership, and advocacy, serving as a beacon of inspiration to nurses at every stage of their professional journey.
Distinguished Career Promoting Professional Nursing:
Katy’s career reflects a profound commitment to advancing professional nursing practice while keeping patient safety and dignity at the forefront. She is unwavering in her dedication to delivering care that is safe, equitable, and grounded in evidence-based practice. Guided by this vision, Katy led organization-wide initiatives that resulted in meaningful and measurable improvements in patient outcomes.
Under her leadership, the organization achieved a 50% reduction in falls with injury through a comprehensive, system-level approach. This included competency validation for all clinical staff, standardized patient communication boards outlining mobility and safety plans, daily audits of fall prevention standards, and multidisciplinary reviews of every event. Her relentless focus on reliability and accountability fostered sustained improvement and a culture of safety.
Similarly, Katy led transformative work in the prevention of healthcare-acquired pressure injuries (HAPIs), resulting in a 40% reduction across the organization. She championed early recognition and rapid intervention strategies, ensured offloading wedges were stocked in every patient room, established turn teams on all units to promote timely repositioning, and implemented routine wound photography to detect subtle changes early. These initiatives exemplify Katy’s ability to translate evidence into practice at scale while maintaining a focus on the patient experience.
Commitment to Professional Development and Empowerment:
Katy’s vision for nursing extends beyond outcomes to the empowerment and development of the nursing workforce. She believes deeply that nurses must be supported and empowered to shape their own practice and lead change. This belief is most evident in her leadership of the organization’s Shared Professional Governance (SPG) redesign, relaunched two years ago.
Since its relaunch, engagement in SPG has increased annually, with 254 staff-driven improvement proposals submitted in 2025 alone. Katy ensures that SPG is truly nurse-led, with staff serving as council chairs and developing content and leading sessions during the 2025 annual SPG Summit. Her commitment to shared decision-making has cultivated both ownership and professional pride among nurses.
The impact of this empowerment is reflected in the organization’s first Nurse Satisfaction Survey, where results exceeded national benchmarks for similar-sized facilities—particularly in RN to RN interaction, nurse opportunities to influence policy, and interprofessional relationships. These outcomes underscore Katy’s belief that when nurses are heard, respected, and engaged, patient care excellence follows.
Role Model, Advocate, and Transformational Leader:
Katy is widely recognized within the broader nursing community as a transformational leader. She is an exemplary, present leader who consistently embodies the Mission, Vision, and Values of her organization through compassionate, servant-hearted leadership. She actively seeks and values the voice of frontline staff, regularly rounding on units and shadowing bedside nurses to gain an authentic understanding of their daily realities.
Katy is a strong advocate for nurses at all levels and partners seamlessly with interprofessional teams to advance nursing practice and improve patient outcomes. She embraces innovation and proactively addresses challenges by creating meaningful forums for connection, including a monthly virtual nursing town hall. Through this platform, she shares updates transparently, invites dialogue, welcomes questions, and fosters trust, inclusion, and professional excellence.
Advocate for Patient-Centered Care and Experience:
At the heart of Katy’s leadership is an unwavering commitment to patients and families. Every initiative she leads is anchored in the belief that high reliability, dignity, and compassion are inseparable from the patient’s experience. Her work consistently enhances safety, communication, and trust—ensuring that care is not only clinically excellent, but deeply human.
Conclusion:
Compassionate, empathetic, and humble, Katy Hoffman leads by example. She does not seek recognition, instead attributing success to the collective efforts of her team and the exceptional nurses she serves. Guided by Jean Watson’s words, “Maybe this one moment, with this one person, is the very reason we are here on Earth at this time,” Katy centers both nursing practice and organizational purpose in every aspect of her work.
For her lifetime devotion to nursing, her transformational leadership, and her enduring impact on patients, nurses, and the profession, Katy Hoffman is profoundly deserving of the DAISY Lifetime Achievement Award.
