Karen was covering for me while I was out on PTO when an urgent referral came to help a patient who needed significant assistance and was going to be homeless.  I had attempted to outreach and follow up with the patient's friend/caregiver several times for resources but they chose not to pursue further.  Karen went above and beyond to ensure the patient was connected with the right resources, which took up her entire day based on the number of outreaches she made to make this happen. Karen was able to reach the Medicaid case worker to schedule an assessment for Long Term Care services.  Karen provided a list of homeless shelters and received confirmation that the patient will be going to a shelter for temporary placement.  

Karen accomplished in one day by herself what normally takes several days and several people collaborating to help someone in an emergent situation that would greatly impact their safety and health.  She exemplifies someone who "lives their mission" daily and treats everyone she outreaches with compassion and respect.  She has the integrity of someone who will do anything and everything that she can do to help someone regardless of time or other constraints, she will work until the situation has been resolved.  She is one of a kind and someone to look up to, the true definition of an amazing nurse and person.
