This honoree was recognized for her response when she went to a patient's home and found him lying on the floor. She quickly took command of the situation while comforting the tearful elderly gentleman who was incontinent of bowel and bladder due to not being able to get up or call for help. She promptly dispatched the emergency response team, who helped to lift him off the floor and to a more secure surface.

She used her clinical judgment and skills to recognize that this patient was not in a safe situation, alone in his own home. She gained his trust while bathing him and assessing him and assisted him to learn of his options, getting him to agree to work with the hospital liaison to facility insurance authorization and skilled care placement. This honoree went the extra mile working with this patient and the interprofessional team to ensure that he was moved to a safe environment with the appropriate support to provide him care.
