Jessica Malone has consistently approached supporting and leading the OhioHealthy (OHy) Care Management Team. She demonstrates leadership qualities and commitment to the role of Manager. Her pledge to the OHy members, team, and continuous process improvement has significantly enhanced the quality of services provided by this department. Examples of this include leading the effort to create a comprehensive program description and workflow, as well as solidifying processes and addressing all issues related to the transition to a new third-party administrator at the same time.

Key Achievements:

1. Patient-Centered Care: Jessica educated her team on constructing individualized care plans leveraging evidence-based Milliman Care Guidelines, which prioritize member needs and outcomes, resulting in improved member satisfaction scores and health outcomes.
2. Team Leadership: Under Jessica's guidance, the care management team has achieved remarkable cohesion and efficiency, fostering a supportive and productive work environment in the midst of change.
3. Process Improvement: Jessica has spearheaded several process improvement initiatives. This includes the enormous effort of implementing a new clinical documentation system (TruCare) for the entire team and over a limited period of time. She was a key stakeholder on the implementation team, developing and leading streamlined workflows, team education, care plan development, and efficient processes. Jessica also instructed the team on how to leverage the system for the best member outcomes through coordination of care, transitions of care, in-network steerage, education, and resource referrals.
4. Professional Development: Jessica is committed to her professional growth, as evidenced by her previous Manager training, and by previously taking on the We Give Steering Committee role to represent OHy. She is also committed to the professional growth of the team, providing ongoing training and mentorship that has empowered the team.
5. Strategic Initiatives: Jessica established clinical high-utilizer meetings in collaboration with the OhioHealth Care Management team to improve care coordination. She expanded the OHy CM team by adding dietitian services to enhance member support. Jessica also leveraged biometric and WebMD Health Risk Assessment (HRA) data from OHES to drive targeted member outreach. Additionally, she helped create a risk stratification model to enable proactive engagement with high-risk members.
