First of all, Jennifer has been a dedicated Intermountain Health employee. She has served in different departments and has been back at the endocrinology clinic for the past 5 years, helping rebuild this much-needed specialty. Jennifer is well-connected in the community. She knows where the resources are and often directs our patients exactly to what they need in a timely manner. Jennifer's care and compassion for all patients cannot be understated. She goes above and beyond to ensure each patient is cared for, feels heard and seen, and has everything they need before they leave the clinic. And when patients call in, she provides prompt responses. We get a lot of wonderful feedback regarding her care from all patients. Jennifer is a beaming example of living in the mountains healthy nation every day. Her dedication and service to the enterprise, to the community, and to those she works with are unparalleled. She is always available to assist.

Jennifer's actions can be seen directly in patient positive outcomes. Most patients get their diabetes screenings completed on time, and A1Cs are generally at goal or near goal when they have interacted with her because she takes patient preferences into consideration and utilizes shared decision-making to come up with the nursing plan that is actionable. For example, she will go the extra mile in ensuring patients requiring assistance in completing patient assistance program applications get what they need done so that they receive the much-needed medications that they otherwise would not afford.

Another example is how she takes the time to listen to the patients during the visits. In this way, patients feel they are a part of a community, and that we care about their wellbeing, not just the disease that they have or whether they are meeting the metrics are not. Jennifer goes out of her way to use her own hard-earned money to always ensure we have adequate healthy snacks in the staff break room. It may seem minimal, but this is a significant contribution that each of the team members truly appreciates. There are lots of ways Jennifer is an amazing individual and a wonderful contributor to Intermountain Health and the community in Grand Junction in general.
