Her excellent communication skills contribute to timely and effective follow-up and care of patients in the Southern Region during this challenging time.
In addition to her clinic care for Pulmonary, Cardiology and ENT patients, Heather provide care management responsibilities for patients who see providers from the Breathing Institute at South Campus, Anschutz and Colorado Springs. After careful consideration, she graciously agreed to provide care management for Dr. Rachel Workman with Integrative Medicine recently.

Dr. Workman’s patients are seen only at NOC locations, which results in some communications falling through the cracks. Heather’s management of these phone calls and MyChart messages has improved communication and consistency for Dr. Workman and her patient families.

Heather also agreed to help cover phones and MyChart messages for Breathing Institute patients in Colorado Springs during a current staffing shortage. Her excellent communication skills contribute to timely and effective follow-up and care of patients in the Southern Region during this challenging time.

Heather’s ability to utilize her assessment skills and nursing process to provide consistent and outstanding care across disciplines and demonstrate clinical excellence in the Pulmonary clinic is a testimony to the flexibility and accountability of ambulatory nurses across the organization.

I am proud to nominate this Extraordinary Nurse for the DAISY Award.
