Heather Luescher, RN

A case manager coordinates and plans care for patients and families. Heather Luescher was the case manager assigned to the patient I cared for that day. Heather and her team demonstrated compassion and kindness by taking the extra steps to advocate and ensure that the wife of the patient I was caring for was able to see her husband before he passed away. This may seem small, but both the patient and his wife had been living in separate assisted living facilities and had not seen each other for about a year. The patient's wife had very little transportation and limited mobility, making this a difficult situation. Heather and the case management team worked diligently to find a way for her to come to the hospital to visit at least one last time. This was a challenging task for the team and meaningful for the patient’s wife. She told me many times how grateful she was that this was done for her. She couldn't write this herself, so I am writing on her behalf to express how grateful she is for all your help.
