Hannah Gurney found a pharmacy that could give a 60-day supply of meds that was affordable to a patient. This patient has been inpatient several times in the past year, with two spinal fusions due to infection. His last admission the patient will again need long-term antibiotic use, however, he has no insurance can cannot afford a third round of IV antibiotics. Hannah was also able to ambulate the patient and coordinate care with the family, who are three hours away. Hannah and the Team could have easily said this is the work of case management and not helped this patient, but they didn't.
