Hannah took it upon herself to make sure the patient was in the best and safest hands possible.
A patient came into the clinic for a routine visit, but it soon became much more. The patient quickly became tearful and divulged that his home health nurse had recently quit and that he had been trying to manage, unsuccessfully, on his own. He noted he had not had a full bath in weeks and that his apartment and clothing were dirty. The patient had previously suffered from multiple strokes, so ADL's were particularly difficult for him. She also found out the patient had a "neighbor" helping him, but further digging made it clear this person was much more of a hindrance than a helper. She was making decisions on his behalf that were stopping a new home health nurse from being able to help him the way he needed.

Hannah took it upon herself to make sure the patient was in the best and safest hands possible. She called social work in to assist and, along with urgent care, made a plan for the patient. He was admitted for the weekend, where we could be assured he was getting proper care. The inpatient social work team was then able to manage his care and ensure a safe home environment. Hannah spent hours communicating with many different team members to coordinate his care. What she did for this patient will benefit him greatly, and without her diligence, he would have remained in an insecure and unsafe home environment.
