What truly sets her apart is her heart.
Gwen demonstrates outstanding dedication, compassion, and unwavering advocacy for her patients. Gwen consistently goes above and beyond her role as a case manager to ensure patients receive safe, appropriate, and dignified care.
Gwen works tirelessly to place patients in facilities that best meet their needs, often navigating complex barriers—especially for unfunded patients. She is diligent, persistent, and never gives up, no matter how challenging the situation may be. Her commitment is evident in the many times she has stayed late to ensure safe discharges, even when a late facility placement is unexpectedly added. She never complains and always follows through, positively impacting both patient safety and length of stay.
What truly sets her apart is her heart. She has gone to patients’ homes to help them complete paperwork when they lacked support, keeps clothing in her office for patients in need, and has personally provided clothes to ensure they leave with dignity. She has even visited patients in facilities after discharge to offer support to those without family.
Gwen is a dependable, compassionate, and exceptional case manager who shines as a true ray of sunshine for both patients and colleagues. She exemplifies the spirit of the DAISY Award and is deeply deserving of this recognition.
