Grant took the time on days he was not seeing the patient to collaborate with the Home Health Interdisciplinary Team to make sure this patient received his very important anti-coagulant medication.
I would like to praise Grant for his efforts with wound healing. Recommending wound products, providing education for increasing wound healing, and providing education for using and applying compression. S would not be anywhere near where he is in healing without Grant's expertise and kindness.

Grant worked on a challenging patient case regarding medication. The patient was discharged from the ED with a medication that was not covered by his insurance. The patient had no way to get to a Pharmacy to pick up the medication, and the cost was beyond the patient’s limited budget. Grant took the time on days he was not seeing the patient to collaborate with the Home Health Interdisciplinary Team to make sure this patient received his very important anti-coagulant medication. Grant reached out to doctors, the VA, and various pharmacies as well as the ED Department to attempt to get the patient his medication at a cost he could afford.
